How do I do html form file uploads?

Following the PHP Documentation on file uploads is all you need to do. An example is contained in the first annotation below.

If you wanted to let anyone upload files to your account, this (or some other web script) is the only way to do it since we do not allow Anonymous FTP.

For file uploads larger than 8MB, you must have these settings in your
php.ini file:

 upload_max_filesize = (some number greater than 8 million)
 post_max_size  = (some number greater than upload_max_filesize)
 memory_limit = (some number greater than the 2 settings above)

If the "post_max_size" configuration setting is not in your php.ini you
need to add it.

Example of a file upload form that works at Modwest:
Begin copying here:
---------------------------------------------------------
<?php

# set this to the path where you want uploads to arrive

$UPLOAD_DIR = "/uploaded_files/";

if( empty($UPLOAD_DIR) )
{
  print("<h3>The \$UPLOAD_DIR variable has not been set at the top of
this file.</h3>");
}
else if(! is_dir($UPLOAD_DIR) )
{
  print("<h3>The directory $UPLOAD_DIR does not exist. You may need to
create it.</h3>");
}
else if( ! is_writable($UPLOAD_DIR) )
{
  print("<h3>The directory $UPLOAD_DIR is not writable.</h3>");
}

########################################################
?>

<form enctype="multipart/form-data"
      action="<?= $PHP_SELF; ?>" method="post">
      <input type="hidden" name="MAX_FILE_SIZE" value="300000">
      Send this file: <input name="userfile" type="file">
      <input name=go type="submit" value="Send File">
</form>

<?php
#########################################################

if($_POST && $_POST["go"] && !empty($_FILES) )
{
  if (move_uploaded_file($_FILES['userfile']['tmp_name'],
               $UPLOAD_DIR ."/". $_FILES['userfile']['name']))
  {
    if(! chmod($UPLOAD_DIR ."/". $_FILES['userfile']['name'], 0644))
    {
       print "Failed to chmod() file.";
    }
    print "<br><b>File is valid, and was successfully uploaded.</b>";
  }
  else
  {
    print "An error occured during the file upload. The error code was:
" .
          $_FILES['userfile']['error'] .
          "<br>Decipher the error code at:
php.net/manual/en/features.file-upload.errors.php";
  }
}
?>
-------------------------------------------------------------------
end copying here.

Note that you should never allow anonymous visitors to upload files to
your account, particularly to a location that is web-accessible. Doing
so creates an opportunity for hackers to install and run malicious
software in your account. Therefore the destination of your uploads
should be inaccessible to web browsers or password protected.

If the example above doesn't work out of the box, check your
/logs/php_error_log or /logs/php5_error_log
